Job summary
BAYADA Home Health Care in Gilbert, PA is seeking a Registered Nurse (RN) – Home Health for ongoing visits. This role offers flexible schedules, 1:1 patient care, and a strong work-life balance in Saylorsburg and surrounding areas.
The RN will follow care plans, administer medications, manage wounds, document care, and coordinate with the Clinical or Client Services Manager. A PA license, minimum one year of clinical experience, and good communication are required.
Qualifications
- Current Pennsylvania RN license and eligibility to practice.
- Minimum of one year of clinical (medical/surgical) experience; home health preferred.
- Graduation from an accredited nursing program; reliable and able to travel.
Responsibilities
- Follow a designated care plan in accordance with patient needs.
- Make home visits to clients in designated geographic territories.
- Administer medications, wound care, treatments, and procedures.
- Monitor clients' conditions; report changes to Clinical or Client Services Manager.
- Document observations, interventions, and evaluations using an EMR system.
- Coordinate case management and referrals for each patient.
- Ensure accurate documentation of care provided and patient progress.
