Job summary

ALC Home Health is seeking a PRN Physical Therapist Assistant to deliver skilled PT services in patients' homes across the Illinois area. You will operate on a flexible schedule, manage visits and collaborate with a Home Health Director using EMR tools.

Requirements include PTA degree, state licensure, CPR, and a valid driver's license with insured vehicle. Two years of PTA experience preferred; excellent communication and organizational skills are essential.

Qualifications

• Graduate of an accredited PTA program with an Associate's degree approved by APTA
• Current license as required by state law
• Two years of PTA experience preferred
• Good verbal and written communication and organizational skills
• CPR certification
• Must have insured, operable vehicle
• Must possess general liability/workman’s comp coverage/certification

Responsibilities

• Provides physical therapy services per physician's plan of care
• Assesss patient status and participates in the total plan of care
• Directs PT treatment
• Instructs patients and families on therapeutic devices
• Identifies needs for other home health services and refers
• Prepares clinical/progress summaries
• Participates in discharge planning
• Provides PT consultation to families
• Delivers in-service education to staff
• Participates in peer consultation
• Supervises PT assistants as policy/state rules
• Instructs and evaluates home health aide care every two weeks
