Job summary

Qualifications

• Master's or Doctoral degree in Physical Therapy from an accredited program.
• Licensed to practice as a Physical Therapist in California.
• Minimum of one year of experience as a Physical Therapist; home health experience preferred.
• Strong verbal and written communication and organizational skills.
• Current CPR certification.
• Licensed driver with a reliable, insured vehicle.

Responsibilities

• Provide physical therapy services per physician's plan of care and state practice act.
• Assess functional status, mobility, gait, and rehabilitation potential.
• Direct and implement therapy treatments and educate patients/families on home programs.
• Discharge planning and coordination with physicians and other care teams.
• Supervise PT aides as required by policy and state regulations.
