Qualifications
- Must hold an active Physical Therapist license in the state of practice.
- Degree from an APTA-accredited program (BS, MS, or DPT).
- Minimum one year of SNF experience; willing to travel to multiple facilities.
Responsibilities
- Evaluate and treat patients with mobility, strength, balance, and functional impairments.
- Create and implement personalized care plans.
- Collaborate with team members to ensure quality outcomes.
- Maintain documentation to meet all regulatory requirements.
- Ensure all physician orders are obtained and up to date.
