Qualifications

• Minimum one year of rehabilitation experience; new graduates welcome.

Responsibilities

• Evaluate and treat adult patients in Home Health per physician orders.
• Collaborate with rehab team to restore function and mobility.
• Document care plans and progress in patient records.
