Job summary

Acentra Health is seeking a PASRR Clinical Evaluator - PRN to join our growing team. This role conducts PASRR evaluations, makes level of care recommendations, and ensures that the quality of care provided meets industry standards and complies with government regulations.

This position is PRN with a flexible schedule and a hybrid mix of telehealth and face-to-face assessments in nursing facilities and hospitals in the Port Angeles, Washington area.

Qualifications

• Active, unrestricted Washington state licensure in healthcare or related field (LPCC/LMHC/LMFT/LCSW/NP/PMHNP).
• Degree in a relevant field from an accredited college or university.
• 3+ years of experience in the assessment and/or diagnosis of mental or psychiatric disorders, including complex differential diagnoses.
• 1+ years of experience with intellectual and developmental disabilities or related conditions.

Responsibilities

• Complete accurate PASRR Level II assessments by reviewing relevant records, gathering information from collateral sources, and evaluating functioning.
• Develop informed level-of-care recommendations and determine the most appropriate next level of care for individuals requiring long-term care placement.
• Review and verify medical records and provide clinical recommendations per regulatory requirements, including psychiatric input, physician notes, and meeting turnaround time standards.
• Interact with healthcare professionals, patients, families, and internal partners to support the assessment process.
• Leverage technology, computer systems, web-based applications, and proprietary platforms to manage assessment activities.
• Ensure timely completion of assessments within scope of practice and in line with contractual requirements.
• Determine a clinical diagnosis accounting for psychological and behavioral issues.
• Assess and analyze psychological, emotional, behavioral, and physical functioning.
