Elite Care Management, Inc. is seeking a dedicated Home Health Physical Therapist (PT) to provide skilled therapy in patients’ homes across Lake County, Indiana. This part-time/PRN, per-visit role offers flexible scheduling and competitive per-visit pay of $75–$80.

Responsibilities include evaluating patients, creating individualized plans, and delivering interventions to improve mobility and safety at home. Assist with education, discharge planning, and coordination with the care الفريق.

Qualificações

• Graduation from an accredited Physical Therapy program is required.
• Current, active Physical Therapist license in Indiana.
• Previous home health experience is required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to travel throughout the assigned service area.

Responsabilidades

• Conduct comprehensive physical therapy evaluations and reassessments in patients’ homes.
• Develop and implement individualized treatment plans based on physician orders and patient goals.
• Provide skilled interventions, including strengthening, gait training, balance, transfers, and functional mobility exercises.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress, adjust plans as needed, and coordinate with the care team.
• Maintain compliance with agency policies and state/federal home health regulations.
