Elite Care Management, Inc. is seeking a dedicated Home Health Physical Therapist (PT) to provide skilled therapy in patients' homes throughout Lake County, Indiana.

This part-time/PRN, per-visit position offers flexible scheduling with competitive per-visit pay and travel reimbursement. The PT will evaluate patients, develop individualized plans, and deliver interventions to improve mobility, balance, and independence while educating patients and caregivers on home exercise programs and safety.

Qualifikationen

• Graduate of an accredited Physical Therapy program.
• Current, active Physical Therapist license in the State of Indiana.
• Home health experience required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to independently manage patient visits and scheduling.
• Reliable transportation, valid driver's license, and ability to travel throughout the assigned service area.
• Proficiency with electronic medical records and mobile documentation.
• Ability to meet the physical demands of providing therapy services in patients’ homes.

Aufgaben

• Conduct comprehensive physical therapy evaluations and reassessments in patients’ homes.
• Develop and implement individualized treatment plans based on physician orders and patient goals.
• Provide skilled interventions, including strengthening, gait training, balance, transfers, and functional mobility exercises.
• Educate patients and caregivers on home exercise programs.
• Provide fall-prevention, home-safety, and body mechanics education.
• Monitor progress, adjust treatment plans as needed.
• Coordinate care with physicians, nurses, therapy staff, and other team members.
• Participate in discharge planning and referrals.
• Maintain compliance with agency policies and home health regulations.
