Job summary

Piedmont Augusta Hospital is seeking a Physical Therapist for Home Health to assess and plan care for patients in their homes. You will evaluate functional status and mobility, determine treatment plans, and perform interventions to improve patients’ well-being, while tracking progress and outcomes.

This role requires a PT license upon hire, CPR certification, and a valid driver’s license with vehicle insurance, as you will travel to patient homes.

Qualifications

• Bachelor's degree in health-related field (biology, exercise science, kinesiology) required.
• Graduate of an accredited CAPTE-accredited program required.

Responsibilities

• Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions.
• Help determine a treatment plan based on assessment; perform interventions to improve patient well-being and evaluate progress.

Education
