Job summary

Life Care Home Health seeks a licensed Physical Therapist to provide coordinated care in clients' homes across Texas. You will plan, implement and evaluate plans of care to restore or maintain patient well-being.

You will conduct PT treatments based on medical referrals, document progress, and communicate with patients, families, and health professionals. Travel to home visits and maintain infection control.

Qualifications

• Two years of PT experience in a clinical care setting.
• Home health experience preferred.
• Excellent interpersonal and communication skills.
• English reading/writing/comprehension required.
• Current state PT license and CPR.

Responsibilities

• Coordinate care plans and assess clients to restore or maintain wellbeing.
• Plan and deliver PT treatment per medical referrals.
• Document plan of care and progress notes accurately.
• Communicate effectively with clients, families, and health professionals.
• Travel to patients' homes and maintain flexible scheduling.
• Ensure compliance with third-party payers and maintain documentation.
• Maintain infection control and safety practices in all patient care activities.
