Job summary

OneWell Health Care is seeking a compassionate Physical Therapist (PT) to provide personalized therapy to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings in Pennsylvania.

You will evaluate mobility, develop goal-based treatment plans, and collaborate with caregivers and other professionals to improve mobility, strength, safety, and overall independence. A flexible per-diem schedule and field-based travel are available to fit your needs.

Qualifications

• PhD/MPH not required; Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• CPR certification preferred or required per policy.
• Strong clinical, documentation, organizational and communication skills.

Responsibilities

• Evaluate mobility, strength, balance, gait, posture and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based PT in home and community-based settings.
• Help individuals improve mobility and independence.
• Recommend mobility aids and adaptive equipment.
• Educate individuals, families and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in meetings and trainings.

As a Physical Therapist, You’ll…:

• Recommend mobility aids, adaptive equipment, therapeutic exercises, and fall-prevention strategies.
• Educate individuals, families, and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.

What We're Looking For:

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• Current CPR certification (preferred or required in accordance with organizational policy).
• Strong clinical, documentation, organizational, and communication skills.
• Proficiency in functional mobility and gait assessments, balance and postural control interventions, adaptive and assistive equipment recommendations, evidence-based physical therapy techniques, and electronic health record (EHR) systems.
• Valid driver's license, a clean driving record, reliable transportation, and willingness to travel to home and community-based service locations. Flexibility to support varying schedules and assigned service areas is required.
• Experience supporting individuals with Intellectual and Developmental Disabilities (IDD) and providing physical therapy services in home or community-based settings is preferred.
• English proficiency is required. Bilingual proficiency (including Spanish) is a plus.
