Job summary

TRUE KARE HOME HEALTH INC is seeking a Home Health Registered Nurse in Florida to provide skilled nursing care in patients' homes. You will perform comprehensive assessments, create and implement individualized care plans, administer treatments, educate patients and caregivers, and collaborate with physicians to achieve optimal outcomes.

Applicants should hold a Florida LPN license (as noted in qualifications), possess CPR/BLS certification, and have strong assessment and communication skills.

Qualifications

• Active Florida LPN license.
• Current CPR/BLS certification.
• Minimum one year of nursing experience preferred.
• Home Health experience is an asset but not required.
• Excellent assessment, communication, and organizational skills.
• Valid Florida driver's license and reliable transportation.
• Ability to work independently while providing compassionate patient-centered care.

Responsibilities

• Conduct comprehensive patient assessments.
• Develop, implement, and update individualized care plans.
• Administer medications and treatments as ordered by the physician.
• Monitor patient progress and communicate changes in condition.
• Provide wound care and other skilled nursing interventions.
• Educate patients and family members on disease management, medications, and home care.
• Maintain accurate and timely clinical documentation.
• Coordinate care with physicians, therapists, and other members of the healthcare team.
• Ensure compliance with Medicare, state, and company policies.
