Job summary

Professional Case Management is seeking Home Health Aides/Certified Nursing Assistants to deliver in-home care in Tennessee. You’ll create your own schedule, enabling you to balance school, travel, volunteering, and family time while providing compassionate, one-on-one care to clients who have served our country.

Ideal candidates will bring CNA certification and be able to perform transfers, vital signs, documentation, range of motion, and activities of daily living.

Qualifications

• Active CNA certification in the state(s) of practice.

Responsibilities

• Provide in-home healthcare services with a focus on patient safety and comfort.
• Build one-on-one relationships with patients in their homes.
• Assist with activities of daily living including light housekeeping, meal preparation and personal hygiene.
• Document care provided and patient status accurately and timely.
• Maintain professionalism and adhere to care plans and medical guidelines.
