Job summary

Rochester Regional Health is seeking an experienced Registered Nurse (RN) to join our Community Health Home Care team in Lakeville, NY. The role focuses on in-home nursing interventions, patient education, and care management to promote good health and one-on-one care in patients' homes.

Responsibilities include comprehensive assessments, care planning, documentation, and collaboration with the interdisciplinary team. Per diem, per need schedule, with flexible days and potential incentives.

Qualifications

• Diploma or associate’s degree in nursing.
• Registered Nurse license in New York State.
• Valid NYS Driver’s License.

Responsibilities

• Provide comprehensive assessments and skilled nursing care in patients’ homes as per physician orders.
• Develop and document individualized care plans with input from the health team.
• Maintain clear communication with patients, families, and office staff about health status and care plans.
• Accurately document client care and physician orders per program standards.
• Adhere to department protocols and applicable regulations.
