Job summary

Open Sky Community Services is seeking a Relief Counselor to support individuals facing mental health challenges in living independently and reaching beyond perceived limitations.

You will help design and implement person-centered plans, assist with community access and transportation, and promote growth and meaningful relationships while documenting all activities. A valid driver's license is required.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record are required.
• Reliability, willingness to learn, and openness to new opportunities are essential.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
