Job summary
Remedy Home Health Care is seeking a dedicated Physical Therapist Assistant (PTA) for home health in Orange County on a per diem basis. You will deliver one-on-one therapy in patients' homes, focusing on rehabilitation, mobility, and fall prevention.

Qualifications
- Active California PTA license required.
- CPR certification (AHA) required.
- Minimum 1 year of clinical home health experience preferred.
Responsibilities
- Provide in-home physical therapy services following the established Plan of Care.
- Assist patients with therapeutic exercises and mobility training.
- Support senior care patients in improving strength, balance, and independence.
- Monitor, document, and report patient progress to the supervising Physical Therapist.
- Educate patients and caregivers on home exercise programs, safety, and fall prevention.
- Collaborate with physicians, RNs, and the interdisciplinary care team.
- Complete documentation using WellSky Kinnser EMR.
