This on-site position supports a multidisciplinary team and offers flexible scheduling, including days and PMs, with PRN hours available to ensure patient coverage.

Qualifications

• Must be a licensed physical therapist in Wisconsin.
• Demonstrate professional attitude and appearance.
• Proficient administrative and communication within department and facility.

Responsibilities

• Uphold professional, legal and ethical standards of practice as defined by APTA and State Practice Act.
• Ensure delivery of quality services and outcomes per department expectations.
• Perform initial patient examination, evaluation, and plan of care; revise as needed.
• Create and maintain patient records.
• Provide on-site assessment and re-evaluation according to required intervals and adjust treatment plan.
