Job summary

Qualifications

Responsibilities

• Evaluate patients and develop or carry out individualized treatment plans (PT), or deliver treatment under supervision (PTA).
• Document evaluations, progress, and outcomes per facility, state, and CMS requirements.
• Collaborate with the interdisciplinary team to support patient goals and discharge planning.
• Educate patients, families, and caregivers on treatment plans and home programs.
• Maintain compliance with all clinical, safety, and regulatory standards.
