River Terrace Retirement Community in Bluffton, IN seeks a licensed Physical Therapist (PT) to join our rehabilitation team on a part-time or PRN basis. You will evaluate residents, create individualized plans, and provide evidence-based therapy to enhance mobility and independence.

The role emphasizes collaboration with interdisciplinary teams, ongoing progress monitoring, and education for residents and families to sustain improvements.

Qualifications

• Licensed Physical Therapist in Indiana (PT)
• Current BLS certification
• Strong clinical judgment and communication skills
• Compassionate, patient-focused approach to care
• Ability to work independently while collaborating with rehabilitation and interdisciplinary teams

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide evidence-based, patient-centered therapy interventions to improve mobility, strength, balance, and functional independence
• Monitor resident progress, update goals, and adjust treatment plans as appropriate
• Collaborate with the interdisciplinary care team to support positive rehabilitation outcomes
• Maintain accurate documentation and ensure compliance with clinical and regulatory standards
• Educate residents, families, and caregivers on exercises and strategies that support continued progress and independence
