Job summary

Continuum Therapy Partners seeks a Speech-Language Pathologist for PRN home health in Stevens Point, WI and nearby areas. You will enjoy the freedom to create your own schedule and provide meaningful one-on-one care within a supportive team environment.

Required: Master's in SLP, Wisconsin license and CCC-SLP; 1 year of experience preferred. You will assess, plan, document progress, and educate patients and families on communication strategies and swallowing safety.

Qualifications

• Master's Degree in Speech-Language Pathology required.
• Current Wisconsin license; CCC-SLP required.
• 1 year of previous experience as a Speech-Language Pathologist preferred.
• Evaluates and develops individualized treatment plans for speech, language, and swallowing disorders.
• Documents patient progress and adjusts treatment plans based on clinical outcomes; Educates patients and families on communication strategies and swallowing safety.
