Transitions At Home for Southern Wisconsin is seeking a PRN Speech Language Pathologist to provide in-home speech therapy services in patients' residences on a flexible schedule. The role works under the supervision of the Director of Clinical Services/Clinical Manager and aims to deliver compassionate, patient-centered care.
Qualified candidates will hold an active Wisconsin SLP license, CPR certification, and a valid Wisconsin driver’s license with car insurance.

An Speech Language Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.
