Qualifications

• Graduate of an accredited nursing program with RN licensure.
• Current state RN license and CPR certification.
• Compassionate, friendly demeanor with excellent communication skills.
• Valid driver’s license and reliable transportation; able to travel locally.

Responsibilities

• Visit patients in their homes for one-on-one assessments.
• Complete initial and ongoing assessments documenting medical history and needs.
• Chart observations, monitor vital signs, and review medications.
• Communicate effectively with patients, families, and health professionals.
• Provide caregiver training as needed.
