Job summary

HealthPro Heritage is seeking a Physical Therapist Assistant (PTA) for a PRN, flexible on-site role at Eliza in Chagrin Falls, OH. You will support resident-focused care in a Long-Term Care setting with a warm, collaborative team.

The position offers a competitive hourly rate ($30–$37) based on experience and a supportive, team-driven environment with well-maintained therapy spaces and engaged residents.

Qualifications

• Associates degree in Physical Therapist Assisting (PTA) from an accredited program.
• Valid state licensure as a Physical Therapist Assistant or licensure-eligible.
• Strong assessment, communication, and interpersonal skills; able to work in a team.

Responsibilities

• Patient care: implement treatment plans designed by Physical Therapists and provide direct care.
• Treatment and education: monitor progress, document observations, educate patients and families.
• Documentation: accurately record treatments, progress, and responses per policies.
• Collaboration: work with Physical Therapists and other staff for coordinated care.
