Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting in Narcoossee, FL. This PRN role offers flexible assignments, the opportunity to help older adults maintain independence, and competitive compensation.

Join a supportive team with clinical autonomy and clear reporting. Most assignments are in-home hospice care, with absent driving between homes, and the ability to select shifts that fit your schedule.

Competenze

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Mansioni

• Evaluate and treat residents in an assisted living community and or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
