Sobe Innovative Rehab in Homosassa, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in a home setting. You will evaluate and treat residents, develop individualized plans, and collaborate with physicians and families to support independence and quality of life.

The role offers a flexible PRN schedule, no driving between homes, and competitive compensation with clinical autonomy and a supportive team.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsabilités

• Evaluate and treat residents in an assisted living community or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
