Job summary

Brushy Creek Post Acute in Greer, SC is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis. Qualifications

• Education: Bachelor's degree in Physical Therapy required.
• Master's or Doctorate in Physical Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Proficiency with Net Health and PointClickCare (PCC) preferred.
• Strong clinical, communication, and documentation skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services per physician orders and goals.
• Assess mobility, strength, balance, endurance, and functional abilities.
• Monitor progress and modify treatment plans as needed.
• Educate residents and families on treatment plans and safety techniques.
