Job summary

Sobe Innovative Rehab in Oakland, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide outpatient therapy in an assisted living community. This PRN position offers flexible assignments and the chance to help older adults maintain independence and quality of life through compassionate, evidence-based care.

You will evaluate and treat residents, develop individualized plans, collaborate with physicians and families, and document in the EMR.

Qualifications

• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
