Sobe Innovative Rehab in Viera, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide therapy services in an Assisted Living Facility and for hospice patients in a home setting. Join our team to help older adults maintain independence and quality of life through evidence-based care.

This PRN role offers flexible assignments, the ability to choose your schedule, and the benefit of working in a single location without driving between homes.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
