Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide skilled therapy services for Hospice Patients in a home setting, in Tavares, FL. The role offers flexible PRN assignments and competitive compensation, with clinical autonomy and a supportive team.

Ideal candidates hold a Florida PT license and a degree in Physical Therapy, with both new graduates and experienced therapists encouraged to apply.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.

Education
