Job summary

Wise Home Care is seeking an experienced Physical Therapist (PT) for PRN Home Health opportunities throughout Richland County and surrounding areas in Ohio. The PT will provide individualized therapy to improve mobility, restore function, reduce pain, and promote independence in the patient’s home.

The role involves evaluating patients, developing and updating plans of care, delivering skilled PT treatments, educating patients and caregivers, coordinating with physicians and the care team, and

Qualifications

• Active Ohio Physical Therapist license required.
• Graduate of an accredited Physical Therapist program.
• Previous Physical Therapy experience preferred.
• Home Health experience preferred.
• Ability to evaluate patients and develop individualized plans of care.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently in a home-based care environment.

Responsibilities

• Evaluate patients to determine functional limitations and appropriate physical therapy needs.
• Develop and implement individualized physical therapy plans of care.
• Provide therapeutic exercise, functional training, mobility and gait training, manual therapy, and other PT interventions.
• Evaluate patient response to treatment and modify plans as needed.
• Establish goals to improve mobility, strength, balance, function and independence.
• Educate patients, families and caregivers on treatment plans and home care techniques.
• Complete evaluations, reassessments, progress notes and discharge documentation on time.
• Communicate patient progress with physicians and care team.
• Coordinate with PTAs involved in the plan of care.
