Wise Home Care in Richland County, OH is seeking a licensed Physical Therapist (PT) for PRN home health opportunities. The PT will provide in-home evaluations, develop individualized plans of care, and deliver therapeutic interventions to help patients regain mobility and independence.

Travel locally to patient homes is required; strong documentation and communication with physicians and the care team are essential.

Qualifications

• Active and unrestricted Physical Therapist license in Ohio.
• Graduate of an accredited Physical Therapist program.
• Home Health experience preferred.
• Ability to evaluate patients and develop plans of care.
• Strong clinical assessment, communication, and documentation skills.
• Must travel locally to patient homes; valid driver's license required.

Responsibilities

• Evaluate patients to determine functional needs and PT requirements.
• Develop and implement individualized plans of care.
• Provide therapeutic exercises, mobility training, and manual therapy.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on plans and home exercise.
• Complete evaluations, progress notes, and discharge documentation.
