Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in home settings around Buena Ventura Lakes, FL. This PRN role offers flexible assignments and the chance to support older adults in maintaining independence and quality of life.

Responsibilities include evaluating and treating residents in home or assisted living settings, developing individualized plans, collaborating with physicians and families, and maintaining EMR documentation.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.

Education
