Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to provide PRN clinical coverage in Galion, Ohio. This role ensures continuity of high-quality care with our full-time team and maintains the same clinical standards and communication voice.

You will participate in evaluating patients, developing individualized plans, and coordinating with a multidisciplinary team. A DPT, active Ohio license, and NPTE are required, with 1 year of PT experience preferred.

Qualifications

• DPT degree required.
• Current, unencumbered state PT license; NPTE required.
• 1 year PT experience preferred.
• Develops and implements individualized treatment plans with other disciplines.
• Documents progress and adjusts plans based on outcomes; educates patients and families.

Responsibilities
