Job summary

Concept Rehab, Inc. is seeking a compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in St. Marys, Ohio. This PRN and part-time position supports older adults in maximizing independence and quality of life.

The COTA will implement treatment plans, guide residents through activities, and work with an interdisciplinary team to ensure high-quality care and compliant documentation. A strong focus on resident outcomes and teamwork is essential.

Qualifications

• Current, unrestricted Certified Occupational Therapy Assistant license in Ohio.
• Graduate of an accredited Occupational Therapy Assistant program.
• Valid driver’s license and reliable transportation.
• Understanding of skilled nursing and long-term care practices, regulations, and documentation requirements.
• Strong communication, organization, and patient-care skills.
• Ability to work effectively within a multidisciplinary healthcare team.

Responsibilities

• Implement individualized treatment plans developed by the Occupational Therapist.
• Guide residents through therapeutic activities designed to improve daily living tasks and functional mobility.
• Monitor resident progress and communicate updates to the supervising therapist.
• Collaborate with an interdisciplinary care team, including Physical Therapists, Speech-Language Pathologists, nursing staff, and other healthcare professionals.
• Assist with documentation and ensure compliance with facility policies and regulatory requirements.
• Foster a positive and encouraging environment that supports resident goals and independence.
