Job summary

Innovate Rehab & Wellness is seeking a Licensed PRN Certified Occupational Therapy Assistant (OTA) for a Skilled Nursing Facility in Ringgold, LA. The role focuses on one-on-one therapy, evaluating residents, and providing OT interventions under supervision. Flexible, as-needed shifts support work–life balance.

Requirements include an OTA degree, NBCOT certification, and a valid state license. Prior SNF experience is preferred; strong communication and teamwork skills are essential to deliver

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
