Job summary

Innovate Rehab and Wellness seeks a Licensed PRN Certified Occupational Therapy Assistant to support residents in a Skilled Nursing Facility in Nevada, MO. Qualifications

• Associate’s degree from an accredited OTA program.
• NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.

Education
