Job summary

Human Touch Home Health in Virginia is seeking a compassionate Occupational Therapist (OT) for PRN home health opportunities across Loudoun County. You’ll conduct in-home evaluations, develop individualized plans of care, and support patients to regain independence in daily activities.

Virginia license required, valid driver’s license, and ability to work independently in patients’ homes. Flexible scheduling and territory selection are offered, with mileage reimbursement and one-on-one patient

Qualifications

• Active Virginia Occupational Therapist license is required.
• Graduate of an accredited OT program.
• Valid driver’s license and reliable transportation.
• Strong clinical assessment and communication skills.
• Ability to work independently in patients' homes.
• Home health experience preferred but not required.
• WellSky experience is a plus.

Responsibilities

• Complete occupational therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Help patients improve independence with activities of daily living.
• Provide therapeutic interventions to improve strength, coordination, mobility, and function.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients and caregivers on safe techniques and strategies.
• Monitor progress and adjust treatment plans as needed.
• Coordinate care with physicians, nurses, therapists, and other members of the care team.
• Complete accurate and timely documentation using WellSky.
• Participate in reassessments and discharge planning.
