Job summary

Always Best Care of Greater Cleveland is seeking a compassionate PRN Occupational Therapist to provide quality home health services to clients in the Greater Cleveland area, with flexible, as-needed scheduling.

The OT will evaluate function, develop individualized treatment plans, and support patients to perform daily activities safely at home, while collaborating with families and the healthcare team.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and valid Ohio Occupational Therapist license.
• Previous home health experience preferred.

Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans.
• Provide occupational therapy interventions to improve daily living skills, mobility, coordination, and independence.
• Assist patients with adapting to their home environment and recommend appropriate assistive devices.
• Educate patients and caregivers on exercises, safety techniques, and strategies for daily activities.
• Monitor patient progress and modify treatment plans as needed.
• Complete accurate and timely clinical documentation.
• Communicate effectively with physicians, nurses, patients, families, and other members of the healthcare team.
• Follow established care plans, clinical guidelines, and regulatory requirements.
