Continuum Therapy Partners seeks an Occupational Therapist for PRN shifts at Trinity Care Center in Farmington, MN. You’ll deliver high-quality care with flexibility to fit your schedule while maintaining the same clinical standards as the full-time team.

Required qualifications include a Master's or Doctorate in OT, current MN OT license, NBCOT certification, and at least 1 year of experience. This role emphasizes independent treatment planning, progress documentation, and patient/family

Formación

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state OT License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
