Job summary

Human Touch Home Health in Pennsylvania is seeking a compassionate Occupational Therapist to provide PRN home health services in Quakertown, Coopersburg, and surrounding Upper Bucks County communities. You will perform in-home evaluations and develop personalized treatment plans.

You’ll educate patients and caregivers, monitor progress, and coordinate with physicians and other team members. A PA OT license, a valid driver’s license, and the ability to work independently are required; WellSky

Qualifications

• Active Pennsylvania OT license required.
• Graduate of an accredited OT program.
• Valid driver’s license and reliable transportation.
• Strong clinical assessment and communication skills.
• Ability to work independently in patients' homes.
• Home Health experience preferred but not required.
• WellSky experience is a plus.

Responsibilities

• Complete OT evaluations in patients' homes.
• Develop individualized treatment plans.
• Help patients improve independence with ADLs.
• Provide interventions to improve strength, coordination, mobility, and function.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients and caregivers.
• Monitor progress and adjust treatment plans.
• Coordinate care with physicians, nurses, therapists, and other team members.
• Complete accurate documentation using WellSky.
• Participate in discharge planning.
