Job summary

LilyCare Hospice in New Hampshire is seeking a Hospice Registered Nurse PRN to provide skilled nursing visits in patients' homes and facilities across the NH territory. This flexible PRN role offers per-visit pay and work across a broad New Hampshire footprint.

The ideal candidate holds an active NH RN license, BLS certification, and at least one year of hospice or home care experience, with a reliable vehicle and proficiency in EHR documentation.

Qualifications

• Active New Hampshire RN license in good standing (compact/multistate accepted)
• Current AHA BLS/CPR certification
• One year of RN experience; hospice or home care experience strongly preferred
• Valid NH driver's license and a reliable, insured vehicle for patient visits
• Strong assessment, end-of-life symptom management, and EHR documentation skills

Responsibilities

• Conduct skilled nursing visits in patients' homes and facilities, assessing symptoms, pain, and comfort needs
• Develop, implement, and revise the plan of care with the interdisciplinary group and attending physician
• Administer medications and provide expert pain and symptom management for patients at end of life
• Educate and emotionally support patients and families throughout the hospice journey
• Document every visit accurately and on time in the electronic health record, meeting visit and regulatory requirements
