Job summary

LilyCare Hospice of New Hampshire is seeking a Hospice Registered Nurse PRN to support patients across the New Hampshire territory with flexible, self-directed visits. You will provide skilled nursing care in homes and facilities, focusing on comfort, symptom management, and compassionate end-of-life support.

You will collaborate with an interdisciplinary team, develop and adjust plans of care, document visits in the EHR, and educate families.

Qualifications

• NH RN license in good standing (compact/multistate accepted).
• Current AHA BLS/CPR certification.
• Minimum 1 year RN experience; hospice or home care preferred.
• Valid NH driver’s license and insured vehicle for visits.

Responsibilities

• Conduct skilled nursing visits in patients' homes and facilities; assess symptoms, pain, and comfort needs.
• Develop, implement, and revise the plan of care with the interdisciplinary team and attending physician.
• Administer medications and provide expert pain and symptom management.
• Educate and emotionally support patients and families during the hospice journey.
• Document visits accurately in the EHR and meet regulatory requirements.

Duties and Responsibilities

• As a Hospice Registered Nurse PRN, conduct skilled nursing visits in patients' homes and facilities, assessing symptoms, pain, and comfort needs
• Develop, implement, and revise the plan of care with the interdisciplinary group and attending physician
• Administer medications and provide expert pain and symptom management for patients at end of life
• Educate and emotionally support patients and families throughout the hospice journey
• Document every visit accurately and on time in the electronic health record, meeting visit and regulatory requirements

Compensation and Schedule

• Paid per visit; rate based on visit/experience
• PRN / per diem — flexible, as-needed scheduling across your New Hampshire territory
• Field-based role serving patients in home and facility settings

Skills and Qualifications

• The ideal Hospice Registered Nurse PRN holds an active New Hampshire RN license in good standing (compact/multistate accepted)
• Current AHA BLS/CPR certification
• One year of RN experience; hospice or home care experience strongly preferred
• Valid NH driver's license and a reliable, insured vehicle for patient visits
• Strong assessment, end-of-life symptom management, and EHR documentation skills
