Qualifications

• Graduate of an accredited seminary or school of theology and/or ordination from a faith group; completion of CPE preferred
• Hospice experience with death and dying strongly preferred
• Comfortable in an interfaith setting and respectful of diverse beliefs
• Valid driver’s license and proof of insurance

Responsibilities

• Perform spiritual bereavement assessments for patients and families and incorporate needs into the care plan
• Provide direct spiritual care and bereavement counseling to patients and families
• Offer compassionate presence and support to patients, families, and the care team
• Document patient services timely and accurately to reflect the plan of care
• Participate in group meetings to coordinate care with the interdisciplinary team
• Provide consultation and education on spiritual care and bereavement to the team
• Support on-call coverage as needed and supervise spiritual care volunteers when assigned
• Direct and implement the bereavement plan of care, offering services up to one year after death
