Job summary

Better at Home is seeking a PRN Home Health RN to travel across Lake, Lorain, Cuyahoga, Medina, and Geauga counties from our Mentor office. You will conduct in-home assessments, develop care plans, and ensure patients meet program criteria, all on a flexible PRN schedule.

You must hold an active Ohio RN license, have reliable transportation, and bring home health or Medicaid experience. This role offers meaningful patient interaction and autonomy in a field setting.

Qualifications

• Active, unencumbered RN license in Ohio.
• Ability to travel extensively within the five-county territory.
• Experience in home health or Medicaid populations preferred.
• Strong clinical judgment and time management; ability to work independently.

Responsibilities

• Conduct in-home initial assessments and routine reassessments.
• Develop and update individualized care plans.
• Navigate territory to reach patients in homes across designated counties.
• Maintain EMR documentation in accordance with guidelines.
• Coordinate with clinical supervisors and physicians to ensure continuity of care.
