Zusammenfassung

CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one, in-home nursing care, develop customized care plans with physicians, and monitor patient recovery. You will educate families, maintain documentation, and coordinate care with a multidisciplinary team.

The role requires RN licensure, current CPR, and reliable transportation to visit patients in their homes. A collaborative, patient-centered environment is offered with Humana backing.

Qualifikationen

• Must have med surg, ICU, ER, or acute care experience.
• Current, unrestricted registered nurse licensure.
• CPR certification current and up to date.
• Experience collaborating with a healthcare team.

Aufgaben

• Provide skilled nursing care in patients' homes, including meds administration and wound care.
• Develop and implement individualized care plans with physicians.
• Monitor patient conditions and report changes promptly.
• Educate patients and families on disease management and treatments.
• Maintain accurate patient care records and coordinate with professionals.
• Report progress to physician and Clinical Manager.
• Oversee Home Health Aide work and ensure proper documentation.

Kenntnisse

Med surg/ICU/ER experience
