Old Dominion Rehabilitation and Nursing is seeking a Physical Therapist to provide skilled rehabilitation aimed at maximizing residents’ mobility, independence, and quality of life. The role supports a resident-centered care philosophy within a collaborative rehabilitation team.

The PT will evaluate residents, develop individualized treatment plans, and work with nursing and medical staff to implement therapy that aligns with physician orders and care plans.

Qualifikationen

• Graduate from an accredited Physical Therapy program.
• Current and valid Physical Therapist license/registration in the state of practice.
• Strong clinical, communication, documentation, and organizational skills.
• Compassionate, patient-focused approach to resident care.

Aufgaben

• Evaluate referred residents and determine their rehabilitation needs.
• Develop and implement individualized treatment plans based on each resident’s needs.
• Provide skilled physical therapy services in accordance with physician orders and plans of care.
• Educate residents and families on PT techniques, exercises, safety strategies, and activities.
• Complete accurate and timely evaluations, progress notes, re-certifications, and discharge summaries.
• Collaborate with nursing, physicians, rehabilitation staff, and other disciplines for coordinated care.
• Maintain confidentiality and comply with HIPAA and applicable regulatory requirements.
