As a Physical Therapist, you will provide skilled, individualized rehabilitation services designed to help residents maximize their physical function, improve mobility, and achieve meaningful therapeutic goals.
Evaluate referred residents and determine their rehabilitation needs.
Assess medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.
Develop and implement individualized treatment plans based on each resident’s needs, capacity, and tolerance.
Establish realistic, measurable, and achievable therapeutic goals.
Provide skilled physical therapy services in accordance with physician orders and established plans of care.
Monitor resident progress and modify treatment approaches as clinically appropriate.
Educate residents and families on appropriate physical therapy techniques, exercises, safety strategies, and functional activities.
Complete accurate and timely evaluations, weekly progress notes, re-certifications, treatment documentation, and discharge summaries.
Collaborate with nursing, physicians, rehabilitation staff, and other disciplines to provide coordinated, patient-centered care.
Maintain confidentiality and comply with HIPAA, Federal Residents’ Rights, and applicable regulatory requirements.
Perform other duties as assigned by the Director of Rehabilitation.
Graduate of an accredited Physical Therapy program.
Current and valid Physical Therapist license/registration in the state of practice.
Strong clinical, communication, documentation, and organizational skills.
Compassionate, patient-focused approach to resident care.
Ability to work effectively as part of an interdisciplinary team.
Long-term care and/or geriatric rehabilitation experience preferred, but not required.
Experience with Case Mix Index (CMI) and PPS scheduling preferred, but not required.
