Qualifications

• Minimum 1 year of home care experience required.
• Must have 1 year of supervisory or management experience.
• Valid NYS driver's license and reliable vehicle necessary.

Responsibilities

• Conduct on-site supervisory and teaching visits to evaluate caregivers.
• Provide clinical orientation and ongoing guidance for assigned home care cases.
• Complete initial nursing assessments and reassessments.
