Job summary

All Metro Health Care, a Modivcare Personal Care Service, seeks a Per Diem RN Supervisor- Home Care to lead on-site care in Binghamton, NY. You will mentor nurses and home health aides, ensure quality documentation, and coordinate services across patient visits.

Responsibilities include nursing assessments, caregiver orientation, staff training, and collaboration with physicians to adjust care plans. A valid NYS RN license and CPR certification are required, with mileage reimbursement provided.

Qualifications

• Registered Nurse required.
• CPR certification required.
• 1 year of Home Care experience required.
• 1 year of supervisory or management experience required.
• Valid NYS Driver’s License, insurance, registration, and reliable car.

Responsibilities

• Performs on-site supervisory and teaching visits to evaluate caregivers.
• Provides clinical orientation to caregivers on assigned home care cases.
• Participates in the training of new nurses and in-services for field staff.
• Performs initial nursing assessments.
• Counsels patient/caregiver in meeting nursing and related needs.
• Assigns and coordinates services provided.
• Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs.
• Prepares clinical progress notes ensuring any changes in the patient’s condition is reported.
• Monitors clinical records for complete and accurate documentation.
