Job summary

Loudoun Rehabilitation and Nursing Center in Leesburg, VA is seeking a dedicated Physical Therapist to join our rehabilitation team in a per diem/casual capacity, with flexible scheduling.

You will perform comprehensive PT evaluations, develop individualized treatment plans, provide skilled interventions to improve mobility and independence, and collaborate with an interdisciplinary team to optimize resident outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and valid Physical Therapist license in Virginia.
• Experience in skilled nursing or geriatric rehab preferred but not required.
• Strong clinical reasoning, communication, and organizational skills.
• Ability to work collaboratively as part of an interdisciplinary team.
• Commitment to compassionate, high-quality patient care.

Responsibilities

• Perform comprehensive PT evaluations to assess medical conditions, functional abilities, limitations, and rehabilitation potential.
• Develop and implement individualized treatment plans with measurable, patient-centered goals.
• Provide skilled therapeutic interventions to improve mobility, strength, balance, safety, and functional independence.
• Monitor progress and modify treatment plans as needed to achieve optimal outcomes.
• Collaborate with patients, families, and interdisciplinary team members to establish realistic goals and coordinated care.
• Educate patients and families on treatment plans, safety strategies, and techniques to support continued progress.
• Complete accurate and timely clinical documentation, including evaluations, progress notes, recertifications, and discharge summaries.
• Participate in interdisciplinary collaboration and facility quality-improvement initiatives.
